IOL optic partial pupil capture: pediatric case

In EnglishEN: A case of August 2016. A case of pediatric cataract surgery from 2009, which later complicated with partial intraocular lins optic dislocation and pupillary capture. The girl was 9, when she was operated on, and this was bilateral juvenile cataract. Now she is 16 yo. She is bilaterally amblyopic of course. The eye with this picture sees better than the other one, where IOL is properly positioned. Due to complexities, which concur with IOL explantation and scleral fixation, due to absence of functioning vitrector at the time, the eye was left intact. Her vision in this eye was 6/18.

“Pupillary capture is a common complication after pediatric cataract surgery, which occurs most often in children under 2 years of age, when an optic size of less than 6 mm is used and the lens is placed in the ciliary sulcus. The IOL optic capture technique involves drawing an IOL optic through a PCCC opening, and apposition of the anterior and posterior capsule leaflets. This maneuver ensures the anterior and posterior capsule leaflets are closely in contact and closed on the front surface of IOL. In this way, the risk for pupil capture is greatly reduced. This is one of important advantages of IOL optic capture. So far, complication of pupillary capture after IOL capture are rarely reported.”

Коллеги, это фото глаза с прооперированной в 2009 г. катарактой. На данный момент пациентке 16 лет, и зрение в этом глазу с коррекцией как ни странно даже лучше чем в парном, куда более красивом глазу (6/18). Трогают ли такие глаза у вас в больнице? Если трогают – то как?

Квинтессенцией совместного решения было следующее. В идеальной ситуации эту линзу нужно вернуть в нормальное положение или заменить на другую, которую уже подшить, например к склере. В данном же случае было несколько но: 1) Это мощный фиброз вокруг гаптик. Застарелое послеоперационное воспаление. Линзу явно имплантировали в борозду (при этом линза из ПММА, одночастная, что как бы в норме делать не рекомендуется). Работающего витреотома на момент случая в клинике не было. Без него трогать этот фиброз страшно. Кроме того, оптику ПММА простыми ножницами разрезать нельзя. Удаление было бы сопряжено с большой травмой. Было решено оставить всё как есть.
